Why your skin probably isn't a skin problem
If you've tried every cleanser, tried cutting out dairy, tried the expensive serum, and your skin still isn't responding the way you'd expect — there's a good reason for that. Persistent skin conditions like acne, eczema, rosacea and hyperpigmentation often have internal drivers that no topical product can reach. The most clinically significant of these is your gut.
What is the gut-skin axis
Research published in Gut Microbes (2025) describes the gut-skin axis as a bidirectional communication system between the gastrointestinal tract and the skin — mediated by the immune system, systemic inflammation, microbial metabolites, and the integrity of both the gut and skin barriers. In plain terms: what's happening in your gut is actively influencing what your skin does. And it works both ways.
Your gut is home to trillions of microorganisms — bacteria, fungi, archaea and viruses — collectively known as the gut microbiome. This microbial community does far more than assist with digestion. It regulates immune function, modulates systemic inflammation, produces vitamins and short-chain fatty acids, and communicates with virtually every organ system in the body, including the skin.
A 2025 review in Cosmetics (Mohsin et al.) identified consistent associations between gut microbiome dysbiosis — an imbalance in the microbial community — and a range of dermatological conditions including acne vulgaris, atopic dermatitis (eczema), rosacea and psoriasis. The mechanisms are well-documented: when the gut microbiome is disrupted, immune dysregulation and systemic inflammation follow, and the skin — as a highly immunologically active organ — reflects that disruption clearly.
A 2024 editorial in Frontiers in Microbiology (Chai et al.) confirmed that gut microbiota play critical roles in modulating the development of skin disorders beyond the gastrointestinal tract, including through microbial metabolites that reach the skin via systemic circulation.
The three mechanisms that matter most
1. Intestinal permeability — "leaky gut" and skin inflammation
When the mucosal lining of the gut is compromised, lipopolysaccharides (LPS), bacterial fragments and undigested food proteins can pass into systemic circulation through what are called "tight junctions." This triggers an immune response and drives low-grade systemic inflammation — the precise inflammatory environment that underpins acne, eczema flares and rosacea.
Intestinal permeability is measurable through functional pathology — I regularly use zonulin levels, Lactulose/Mannitol testing and comprehensive stool analysis to assess the integrity of the gut barrier in clients presenting with chronic skin conditions. Identifying and repairing intestinal permeability is often the single most significant intervention in a skin case.
2. Microbial metabolites and short-chain fatty acids
A healthy, diverse gut microbiome produces short-chain fatty acids (SCFAs) — particularly butyrate, propionate and acetate — through the fermentation of dietary fibre. These compounds regulate immune cell function, reduce pro-inflammatory cytokine production, and support skin barrier integrity. When microbial diversity is low and SCFA production is reduced, the skin loses one of its key systemic anti-inflammatory inputs.
This is one of the key reasons why dietary interventions — not simply probiotic supplementation — are central to effective gut-skin treatment. Feeding the microbiome the substrates it needs to produce SCFAs requires a diet high in diverse plant foods, resistant starches and fermented foods.
3. The HPA axis — stress, cortisol and the skin
The gut-skin axis doesn't operate in isolation. It intersects with the hypothalamic-pituitary-adrenal (HPA) axis — the body's central stress response system. Chronic psychological stress alters gut microbiome composition, increases intestinal permeability, and elevates cortisol — which in turn drives sebum production, disrupts the skin's immune surveillance, and impairs wound healing. This is why stress reliably worsens acne, eczema and rosacea in most patients, and why stress management isn't a nice-to-have in a skin treatment plan — it's a clinical priority.
What this means in practice
In my clinical work, I treat skin as a systemic condition first and a local condition second. For a client presenting with persistent acne or eczema, the initial investigation covers:
- Comprehensive digestive stool analysis (CDSA) to assess microbial diversity, pathogens, inflammatory markers and digestive function
- Intestinal permeability markers
- Food sensitivity and IgG testing where indicated
- Inflammatory markers (hs-CRP, homocysteine)
- Hormonal assessment, particularly androgens and oestrogen metabolism in women with acne
- Nutritional status — zinc, vitamin D, omega-3 index and B vitamins are frequently deficient in skin conditions
The treatment protocol that follows is built around three pillars: repairing the gut barrier, restoring microbial diversity, and reducing the systemic inflammatory load. Topical Ocinium cosmeceuticals are prescribed as part of the protocol — not instead of it — because the most effective results come from addressing the skin from the inside and the outside simultaneously.
Conditions most strongly linked to the gut-skin axis
Acne vulgaris — gut dysbiosis, elevated androgens and insulin resistance are frequently co-occurring drivers. Research consistently identifies altered gut microbiome composition in acne patients, with reduced microbial diversity and lower SCFA-producing bacteria compared to controls.
Atopic dermatitis (eczema) — one of the most well-studied gut-skin conditions. Early-life gut microbiome composition is now considered a primary determinant of atopic dermatitis risk. Dysbiosis, increased intestinal permeability and Th2-skewed immune responses are consistently identified.
Rosacea — multiple studies have identified higher rates of small intestinal bacterial overgrowth (SIBO) and Helicobacter pylori infection in rosacea patients compared to controls. Treating SIBO in rosacea patients has been associated with significant symptom reduction.
Psoriasis — systemic inflammation driven by gut dysbiosis is a central mechanism. Research published in Frontiers in Microbiology (2024) confirmed the gut microbiota's role in modulating psoriatic disease via immune and metabolic pathways.
What you can do now
While a full naturopathic assessment is the most effective way to identify your individual gut-skin drivers, there are evidence-informed foundations that support gut and skin health broadly:
Diversify your diet. Aim for 30 or more different plant foods per week — the diversity of your gut microbiome tracks closely with dietary plant diversity. This means varied vegetables, fruits, legumes, nuts, seeds and whole grains, not just eating more of the same foods.
Include fermented foods daily. Kefir, yoghurt (full-fat, unsweetened), kimchi, sauerkraut and miso all provide live bacterial cultures that support microbial diversity.
Reduce ultra-processed food and refined sugar. These are the primary dietary drivers of gut dysbiosis, increased intestinal permeability and the insulin spikes that drive androgen-mediated acne.
Prioritise sleep. Sleep deprivation measurably alters gut microbiome composition within days, and increases inflammatory cytokine production — both of which show up in the skin.
Address stress with structure. Not stress reduction as an aspiration, but as a clinical intervention. Regular exercise, sleep consistency, breathwork and nervous system regulation all directly influence gut microbiome health through the gut-brain axis.
A note on testing
I want to be direct about something: supplements alone rarely resolve chronic skin conditions. The well-intentioned collagen powders, zinc tablets and probiotic capsules available at health food stores are rarely targeted to your individual biochemistry, and are unlikely to address an underlying condition like SIBO, intestinal permeability or significant microbial dysbiosis without testing to confirm what's actually happening.
Functional pathology removes the guesswork. It tells you whether you need to repair intestinal permeability or address a specific bacterial imbalance, whether your omega-3 status is genuinely deficient, and whether there's an underlying hormonal driver in your skin. From there, a protocol can be built that's specific to you — not just generic advice dressed up as personalised care.
This inside-out approach to skin health is something I've developed over more than a decade of clinical practice, including many years as a skin naturopath in Sydney where I worked extensively with women presenting with treatment-resistant acne, eczema and rosacea. The consistent pattern I observed was that the cases that failed to respond to topical treatment almost always had a gut or hormonal driver that hadn't been identified — let alone addressed.
